Healthcare professionals often face ethically complex cases in end-of-life care, informed consent, and surrogate decision-making, which can result in significant moral distress. This paper introduces the Bioethics Artificial Intelligence Advisory (BAIA) framework, an agentic AI system designed to support clinical ethical decision-making. BAIA integrates multiple bioethical approaches—principlism, casuistry, and narrative ethics—with advanced AI capabilities to deliver context-aware, actionable insights. The framework employs structured methodologies such as paradigmatic case review, mattering maps, and scenario-based reasoning. Case studies involving end-of-life care and conjoined twin separation highlight BAIA’s potential to harmonize diverse ethical perspectives while alleviating clinicians' moral burden. BAIA’s agentic architecture also enables integration with existing systems like METHAD, Delphi, and EAIFT. The paper further addresses limitations related to bias, data quality, and the necessity of maintaining human oversight, advancing responsible AI for ethical healthcare.
