Endoscope-assisted anterior odontoid screw fixation for odontoid process fracture
Abstract
Odontoid fractures in the elderly often lead to substantial morbidity and mortality. This study explores the outcomes of water-based endoscope-assisted anterior odontoid screw (AOS) fixation, a minimally invasive technique, in managing these fractures.
Six patients (5 men, 1 woman) underwent endoscope-assisted AOS fixation for displaced odontoid fractures. Surgical procedures, follow-up assessments were reviewed retrospectively.
Inclusion criteria comprised Type IIa or Type IIb odontoid process fractures with potential non-union/instability. The mean operation time was 82 minutes (69-107 minutes), with an average estimated blood loss of 28mL (10-50mL).
Among the five patients followed for more than 6 months, four (4/5, 80%) showed successful fusion through computed tomography. Screw-related complications were observed in one patient. AOS fixation is advocated for managing unstable odontoid fractures, providing strength and fracture union rates without impending cervical motion. Incorporating minimally invasive approaches, particularly endoscopic techniques, has garnered interest.
The novel endoscope-assisted AOS fixation method, unlike prior approaches, begins with endoscopic guidance and minimal incisions, potentially reducing the risk of neurovascular injury. This novel technique shows promise as an effective strategy for addressing unstable odontoid fractures.
Biography
Kwang-Ryeol Kim has completed his B.D. at the age of 24 years from Keimyung University, Republic of Korea in 2010 and M.D. at the age of 27 years from same University. He is currently pursuing his PhD at Kyungpook National University in South Korea, starting in 2024. He completed his residency in Neurosurgery department at Keimyung University Dongsan Medical Center(DSMC) until 2015. He served as a Military Doctor from 2015 to 2018. He completed a Spine fellowship at DSMC in 2018 and a Spine fellowship at Gangnam Severance Hospital in 2019. In 2020, he began his career as an assistant professor at the Catholic Kwandong University, International St. Mary’s Hospital, Republic of Korea. In 2023, he moved to Daegu Catholic University Hospital, Republic of Korea. He is the Spine specialist of Neurosurgery department of Daegu Catholic University, Republic of Korea. He has 24 publications that have been cited over 199 times, and his publication h-index is 8. He has been serving as an reviewer of several reputed journals.
